Title: Attachment Upload Bug

I've spotted an issue with attachment uploads for new topics and posts, most likely as the use the same code to attach files.

I attach a file called 'img1.png' and then attach a second image called 'img1.png' by accident so I remove the first one and attach another image called 'img2.png' and submit my post.

I should have 'img1.png' and 'img2.png' attached but I actually have 'img1.png' and 'img1,png' attached.

Hmm, that could be either a flaw in the underlying logic or in the multi-attach plugin on top (yes, *that*'s a plugin, and no, no files were harmed in the operation of that plugin)
